Essential element 6: Review and improve comprehensive care delivery
Reviewing the delivery of comprehensive care is important for ensuring patients are receiving care that meets their clinical and personal needs; that risks are efficiently and effectively identified and mitigated; that the agreed comprehensive care plan is achieving what it aimed to; and that patient goals and expectations are being met.
The way that comprehensive care is delivered should be reviewed at the end of the care episode but it may also need to be reviewed during the episode of care if there are changes in the patient’s condition, expectations, needs, diagnoses or prognosis, or if the care plan is ineffective.
The way comprehensive care is delivered needs to align with the comprehensive care plan, acknowledging that health care is an iterative process. The workforce needs to be agile and responsive to changes in the patient’s needs and context, and modify and adapt the comprehensive care plan and the way that care is delivered to meet those changing needs.
The Commission has developed a paper to support understanding of the type of action clinicians and health service organisations can take to support the review and improvement of comprehensive care delivery.
