How to take a BPMH – resources and tools

The SOP uses a systematic process to obtain a best possible medication history (BPMH), which is an accurate and complete list of all medications taken before admission to hospital.

AMR is when an antimicrobial is no longer able to effectively treat an infection. This is that a microorganism has developed resistance to an antimicrobial, so it won’t work to kill the infection.
