What you'll find in My Health Record
The My Health Record system collects documents from a range of healthcare providers. Some of this information may not be the most up-to-date patient data at the time of access.
Types of content held in a patient’s My Health Record
It’s important to always check the accuracy and currency of the available patient information within the My Health Record, using multiple sources to inform your decision making.
Clinical information
- Medicines information - Sorts and displays medicines information held in a patient’s My Health Record documents in date or alphabetical order.
- Event summaries - Captures key health information about a specific health event that is relevant to the ongoing care of an individual.
- Discharge summaries - May contain information to inform patient history taking.
- Pathology and diagnostic imaging reports - Can be used to find previous pathology and diagnostic imaging results. Helpful for avoiding duplicate testing.
- Specialist letters - May be used to inform a comprehensive patient history or reveal previous specialist reviews.
- eReferrals - A tool to refer a patient to a Specialist or other healthcare provider.
- Shared health summaries - Useful for accessing summary information documented by a patient’s nominated healthcare provider. This may include past and current conditions, medicines information, allergies/adverse reactions and immunisations.
- Prescription & dispense records - Helpful for conducting medication reconciliation and reviewing a patient’s dispensing history.
Consumer-entered information
- Advance care plans - May help to inform decision making for end-of-life care.
- Next-of-kin details - Contains next-of-kin information and contacts.
- Emergency contact details - Contact details of a consumers nominated next of kin.
- Child development notes - Lets consumers to keep track of a child’s early health, growth and development.
- Personal health summaries - Consumers can enter information into a personal health summary including medicines information and allergy and adverse reaction information.
Medicare and PBS data
- Immunisation records - Immunisation information extracted from the Australian Immunisation Register (AIR).
- Organ donation information - Information about a consumer’s organ donation decisions form the Australian Organ Donor Register.
- Medicare claims information - Information extracted from Medicare interaction data.
- PBS claims information - Medicines information extracted from pharmaceutical benefits scheme data.
