Templates 

The Commission has developed a range of templates to support hospitals implementing the A-HSOPS 2.0. These can be modified to meet hospitals’ needs.

The templates include:

  • Project plan
  • Posters 
  • Flyers 
  • Sample communications about the survey. 

These templates will be added to overtime – please send any additional materials you develop and would like to share to indicators@safetyandquality.gov.au.  

Technical specifications 

These specifications are intended for use by hospitals implementing the A-HSOPS 2.0. The specifications are designed to enable consistent collection of the survey, increase response rates and minimise invalid or biased responses.
