The National Medication Management Plan (NMMP) is a standardised form to improve the accuracy and completeness of documented information to support continuity of medication management and medication reconciliation during transitions of care. The user guide has been developed with the support of the Health Services Medication Expert Advisory Group incorporating a number of elements originally devised by WA Health.
