Literature review on safety culture assessment modes
The review of the Australian Health Service Safety and Quality Accreditation scheme considered a range of options to improve the scheme, including the inclusion of patient safety culture assessment. The Commission contracted a literature review to support this policy development. The decision was made to not include any new requirements to use a particular tool to assess patient safety culture. The information in this review was used to help guide the work on local monitoring patient safety culture.

The survey included in the safety culture measurement toolkit is based on a survey developed by the Agency for Healthcare Research and Quality – the Hospital Survey on Patient Safety Culture Version 2. The Agency publishes a bibliography of articles published on their surveys of patient safety culture and supplemental items. The bibliography includes references on the use of the surveys; psychometric analyses; how to improve patient safety culture, analyses linking the survey to outcome data; analyses linking composite measures with site and/or respondent characteristics, and reviews of instruments or studies.
